internal/{etl,fetch,proxy}: distinguish between resolved and requested versions
Functions in internal/etl, internal/fetch, and internal/proxy are
updated to distinguish between resolved and requested versions.
A requested version that cannot by resolved the by proxy info endpoint
is not inserted into module_version_states.
fetch.FetchResult is refactored to add HasIncompletePackages as a field.
